How SCPCN Simplified Data Collection from 70+ Clinics with a Secure Power Pages Portal

South Calgary Primary Care Network (SCPCN) supports 70+ member clinics and more than 300 healthcare professionals who provide care across the region. As part of their operational model, SCPCN collects monthly clinic-reported data on patient visits, screenings, and access metrics, then turns it into meaningful reporting for physicians and provincial stakeholders.

But until recently, that reporting process relied on an aging custom-built portal that posed growing risks to the organization.

“The original system had been built back in 2016 by a third-party vendor, and it did exactly what it needed to do at the time,” says Bryce Liesemer, Manager of IT Operations and Business Continuity at SCPCN. “But more recently, support for the aging system became very challenging.”

Replacing a Risky Legacy Tool with Modern Functionality

The original platform was a custom-built application that clinics used to manually submit physician panel data. That information was used to inform clinical decisions and generate reports for Alberta Health Services.

But as the platform aged, the cracks began to show. Logging in and password resets were a challenge for many users, especially physicians who were already strapped for time. The interface was aging, support tickets piled up, and the back-end system created growing risk for SCPCN’s IT team.

SCPCN needed to modernize without losing critical functionality. The replacement platform had to:

  • Be user-friendly and intuitive for busy clinical staff
  • Be accessible to clinic users outside SCPCN’s Microsoft 365 tenant
  • Support high levels of security, including Canadian data residency
  • Be compatible for Single-Sign On (SSO) through SCPCN’s physician engagement portal
  • Eliminate dependence on a third-party vendor for support

That’s when SCPCN turned to Convverge.

“We had such a good experience with our previous intranet project with Convverge that we went straight to them with this one,” says Bryce. “They listened to what we needed, then came back with two clear, well-thought-out options—SharePoint or Power Pages. We hadn’t even considered Power Pages before, but they showed us what was possible.”

Choosing Power Pages for Flexibility, Security, and Ease

Though a SharePoint solution would have been a lighter investment, Convverge’s team was candid about its limitations. The custom functionality SCPCN needed wasn’t fully achievable in SharePoint without major compromises.

Ease of use was the number one priority for our users. Power Pages gave us that, and more. It cost a little more, but we knew we’d be far happier with the result—and we were right.

Built with tight timelines and complex requirements, the new solution—named the Medical Home Hub—launched successfully just a few months later.

Intuitive for Clinics, Secure for SCPCN

Built on Microsoft Power Pages, the Medical Home Hub gives clinic panel managers and support staff a secure and easy way to submit key data on behalf of SCPCN’s member clinics each month.

Single Sign-On (SSO) makes it seamless to log in through their existing member portal, reducing password fatigue and login issues. Users only see data relevant to their clinic, with strict access controls in place. And while the system may temporarily receive limited patient data (via Excel uploads), all files are automatically purged after processing to meet data residency and privacy requirements.

The interface is really, really easy to use—which is something we knew we had to hit out of the park. We haven’t had any complaints. And in IT, sometimes no news is good news.

Built to Adapt to Changing Reporting Requirements

In addition to simplifying access and data submission, the new portal also gives SCPCN the flexibility to adapt to changing reporting requirements without going back to a developer each time.

“With the old system, every little change meant a back-end update. Now, we have an administration tool that lets us change field values, submission windows, and other settings on the fly. That’s been a game-changer.”

From reducing support tickets to streamlining clinic workflows, the impact has been immediate. Clinic staff spend less time wrestling with logins and forms and more time with patients.

Designed for the People Who Use It

A thoughtful design process is what set this project up for success. Convverge worked closely with SCPCN from the start, using high-fidelity prototypes to explore user flows, refine the interface, and align stakeholders before a single line of code was written. That early design investment saved time during development and ensured the solution was tailored to real user needs.

As SCPCN continues to evolve, the Medical Home Hub offers a secure, adaptable foundation to support future reporting needs.

This was a monster project with insanely tight timelines, and Convverge delivered. They’ve been excellent—honest, responsive, and collaborative. It’s not always easy to find a reliable vendor, but these guys, I’d recommend to anyone.
